Summary Stella Maris, located in Timonium Maryland, is looking for a Home Hospice Intake Nurse to join their growing team. This is an exciting opportunity to work alongside dedicated hospice clinicians to meet the needs of clients in the community we serve.
Under the direction of the Home Hospice Director, the main function of this position is to provide intake & referral services to our Home Hospice patients and their families.
Home Hospice Intake Nurse RN or LPN Monday - Friday Responsibilities This position provides services to home hospice peers, patients and families. Assumes responsibility for required documentation in compliance with State and federal regulations.
- Assists in ensuring that nursing care is provided to all patients: supports the care planning process, assists in coordinating and conducting patient/family meetings, ensures ongoing communication with medical practitioners and conducts regulatory required assessments.
- Contributes to the efficient operation of the department by assuming flexible hours as needed
- Maintains effective interdepartmental and interdepartmental working relationships through formal and informal meetings, problem-solving sessions and staff meetings
- Coordinates admissions and readmissions for home hospice and IPU.
- Responsible for promptly answering incoming calls and providing information and problem resolution in a friendly, professional and knowledgeable manner.
- Data entry of new patient information into the electronic medical record.
- Provides effective communication with patients, staff, other health care professionals and referral sources.
- Calls patients/families to verify demographic information, primary care physician, and to schedule admission visit date.
- Monitors staff productivity and works with clinical managers as needed to assure efficient use of staff and prompt admission of patients.
- Compiles admission packets, uploads into AllDocs, and sends to clinical staff as needed.
- Cross trains in other office staff positions to fill in as necessary.
- Continually monitors phone, fax, e-mail, portals, and other electronic media for patient referrals throughout the day.
- Assures timely completion of individual staff schedules.
- Reviews incoming referrals to ensure agency can meet the clinical needs of the patient.
- Complete insurance verification, initial authorizations, and other intake processes prior to patient admission. Completes insurance verification and ongoing authorizations as needed.
Requirements - RN or LPN license in Maryland
- 2-3 years of hospice intake experience preferred
